Please use this form to withdraw from lessons with Littleton Guitar School. This will ensure that all tuition withdrawals stop as agreed upon in the lesson agreement.
This form must be completed on or before the 15th day of the month to ensure auto-payment stoppage at the end of the current month. All withdrawal submissions made after the 15th will be charged for the following month tuition.
